Job Summary:We are seeking a skilled and compassionate RN Hospice Care Coordinator to join our team. As a RN Hospice Care Coordinator, you will play a vital role in providing high-quality care to patients and their families facing terminal illness and bereavement.
Key Responsibilities:- Coordinate care for patients and families, ensuring continuous assessment of their needs and implementation of the interdisciplinary plan of care.
- Provide education and training to patients, families, caregivers, and referral sources in the care of the patient, including pain management.
- Participate in patient care meetings and weekly team meetings to ensure effective communication and collaboration.
- Develop and coordinate individualized plans of care with the interdisciplinary team.
- Obtain physician orders when necessary and provide those services requiring substantial and specialized nursing skill.
- Delegate nursing tasks to Hospice Aides, Volunteers, and Licensed Practical/Vocational Nurses as appropriate.
- Participate in Quality Assessment and Performance Improvement (QAPI) Program and related activities.
- Develop and achieve professional growth goals and objectives personally and per Team Manager reviews.
We are seeking a RN with 1+ year of experience in hospice care and homecare. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
