Key Responsibilities
  • Case Management Services: Provide comprehensive case management services to patients, including assessment, planning, implementation, and evaluation of individualized treatment plans.
  • Collaboration with Healthcare Providers: Work closely with healthcare providers and community resources to ensure patients receive timely and appropriate care.
  • Field-Based Services: Occasionally, provide field-based services, including home visits and in-field services, as needed.
  • Documentation and Reporting: Maintain accurate and comprehensive progress notes and documentation in patient records.
  • Communication and Coordination: Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
Requirements
  • Experience: 1 year of experience in a behavioral healthcare setting.
  • Education: High school diploma or equivalent required.
  • Skills: Strong communication and interpersonal skills, ability to work in a fast-paced environment, and proficiency in electronic medical records.
  • Language: Bilingual English/Spanish preferred.
