Job Summary

We are seeking a highly skilled and experienced Advanced Practice Provider to join our Home Hospital team. The ideal candidate will have a strong background in acute care and a passion for delivering innovative, patient-centered care in a home-based setting.

Responsibilities
  • Provide acute-level care to patients in their homes, including evaluation, management, and treatment.
  • Collaborate with multidisciplinary teams to identify patients who are eligible for Home Hospital services and facilitate the admission process.
  • Establish relationships with inpatient teams to ensure seamless transitions of care.
  • Communicate effectively with patients, families, and healthcare teams to ensure safe and effective care delivery.
  • Participate in quality measurement and quality improvement initiatives to advance the work of Home Hospital and other alternative care pathways.
Qualifications

To be successful in this role, you will need:

  • At least 3 years of experience as a Nurse Practitioner or Physician Assistant, with a minimum of 2 years of acute care experience.
  • Current Massachusetts licensure as a NP or PA, with a Master's Degree required.
  • Advanced Cardiac Life Support (ACLS) certification.
  • Excellent communication and interpersonal skills.
  • Able to work independently and as part of a team.
Working Conditions

This is a part-time role, 24 hours per week, with rotations for days, weekends, and holidays. Local travel between patient homes within a defined 10-20 mile radius is required.
