As a Medical Home Hospital Advanced Practice Provider, you will play a critical role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes.
Key Responsibilities:- Evaluate and manage patients in their home setting, either in-person or virtually, as a key member of the Home Hospital treatment team.
- Collaborate with multidisciplinary teams, including nurses, coordinators, and Attending Providers, to identify patients who qualify for admission or transfer to the Home Hospital service.
- Establish relationships with inpatient Brick and Mortar teams to ensure seamless transition of care to the Home Hospital Service line.
- Communicate with patients and families about Home Hospital and care plan progression, incorporating patient and family values and goals of care.
- Maintain a safe care environment for HH patients and lead in the practice of and reinforcement of a strong safety culture among all clinical partners involved.
- At least 3 years of Nurse Practitioner (NP) or Physician Assistant (PA) experience with a minimum of 2 years of Acute Care Inpatient &/or Emergency experience.
- Current Massachusetts Licensure as a NP or PA with a Master's Degree required.
- Advanced Cardiac Life Support (ACLS) certification is required.
- Flexible and comfortable with a fast-paced, evolving clinical environment & autonomy.
- Motivated, collaborative team player who also works well independently.
Full-time, 36 to 40 hours per week, with rotation of weekends (no more than every 3rd weekend) and rotation of major holidays. Local travel between patient homes within a defined 10-20 mile radius required.
