Job Summary

We are seeking a highly skilled and experienced Advanced Practice Provider to join our Medical Home Hospital team at Massachusetts General Hospital. As a key member of our clinical team, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Key Responsibilities

  • Provide acute-level care to medical patients in the home setting, including evaluating patients, establishing treatment plans, and collaborating with multidisciplinary teams to ensure seamless care transitions.

  • Work closely with our Home Hospital team to identify patients who qualify for admission to our Medical Home Hospital service and complete the admission process.

  • Establish relationships with inpatient teams at our five participating hospitals to foster seamless transitions of care to our Home Hospital service line.

  • Partner with our remote overnight Home Hospital Nocturnist APPs to provide cross-coverage for patients on our service.

  • Collaborate with multidisciplinary teams and patients' longitudinal care teams to ensure safe, effective, and timely care delivery for Home Hospital patients.

  • Communicate with patients and families about their care plan progression, incorporating their values and goals of care.

  • Maintain a safe care environment for Home Hospital patients and lead in the practice of a strong safety culture among all clinical partners.

  • Attend relevant team and other meetings related to Home Hospital and related work.

  • Participate in quality measurement and quality improvement initiatives to advance the work of Home Hospital and other alternative care pathways.

Requirements

  • At least 3 years of experience as a Nurse Practitioner (NP) or Physician Assistant (PA) with a minimum of 2 years of acute care inpatient and/or emergency experience.

  • Current Massachusetts licensure as a NP or PA with a Master's degree required.

  • Advanced Cardiac Life Support (ACLS) certification required.

  • Flexible and comfortable with a fast-paced, evolving clinical environment and autonomy.

  • Motivated, collaborative team player who also works well independently.

  • Excellent oral and written communication skills.

  • Familiarity with EPIC and TEAMs a plus.

  • Demonstrated knowledge of quality measurement and quality improvement.

  • Track record of proficiency in managing relationships and building networks.

  • Previous experience with Continuous Quality Improvement efforts and development of evidence-based protocols a plus.
