About the RoleWe are seeking a highly skilled and experienced Advanced Practice Provider to join our Home Hospital team. As a Medical Home Hospital Advanced Practice Provider, you will be responsible for providing acute-level care to patients in their homes, working closely with a multidisciplinary team to ensure seamless transitions of care and optimal patient outcomes.
Key Responsibilities- Provide acute-level care to patients in their homes, including evaluating and managing patients, establishing treatment plans, and following Home Hospital protocols.
- Work collaboratively with the Home Hospital team, including nurses, paramedics, and administrative coordinators, to identify and admit patients to the Home Hospital service.
- Establish relationships with inpatient teams at all five MGB sites to ensure seamless transitions of care.
- Participate in daily collaboration with Attending physicians to ensure closed-loop communication about patient care plans.
- Communicate with patients and families about Home Hospital and care plan progression, incorporating patient and family values and goals of care.
- At least 3 years of experience as a Nurse Practitioner or Physician Assistant, with a minimum of 2 years of acute care inpatient and/or emergency experience.
- Current Massachusetts licensure as a NP or PA with a Master's Degree required.
- Advanced Cardiac Life Support (ACLS) certification is required.
- Flexible and comfortable with a fast-paced, evolving clinical environment and autonomy.
- Motivated, collaborative team player who also works well independently.
