Overview:
The Home Hospital Advanced Practice Provider will be part of a clinical team that provides acute-level care to patients in the comfort of their own home. This innovative program aims to improve patient care while increasing hospital capacity and reducing total costs of care.
Key Responsibilities:
- Provide acute-level care in the home setting, including receiving patients, establishing treatment plans, and following Home Hospital protocols.
- Collaborate with multidisciplinary Home Hospital team members and patients' longitudinal care team members to ensure safe, effective, and timely care delivery.
- Communicate with patients and families about Home Hospital, incorporating their values and goals in the plan of care.
- Maintain a safe care environment for Home Hospital patients and lead in the practice of a strong safety culture.
- Participate in quality measurement and quality improvement initiatives to advance the work of Home Hospital and alternative care pathways.
Qualifications:
- At least 3 years of Nurse Practitioner (NP) or Physician Assistant (PA) experience, preferably in an acute hospital setting.
- Current Massachusetts Licensure as a NP or PA with a Masters Degree required.
- Advanced Cardiac Life Support (ACLS) certification is required.
- Flexible and comfortable with a fast-paced, evolving clinical environment.
- Motivated, collaborative team player who also works well independently.
Working Conditions:
The role is full-time or part-time and includes rotations for days, weekends, and holidays. Local travel between patient homes within a defined 10-20 mile radius is required.
