Job Summary

This role focuses on providing high-quality nursing care services in accordance with physician orders. As a Clinical Nurse Leader, you will be responsible for assessing, planning, implementing, and evaluating the care needs of a designated group of patients. You will also provide nursing care and identified clinical support tasks within the scope of practice for achievement of the patient's plan of care as directed.

Key Responsibilities
  • Provides leadership within nursing by providing direction and priority setting in regards to patient care.
  • Develops and maintains collaborative relationships between all members of the Health Care Team inclusive of administrative, clinical, research and inpatient partners.
  • Acts as a resource for all staff in regards to education, planning, relocating and development of evidence-based, best practice clinical programs pertaining to the specialty or primary care patient population.
  • Mentors and actively participates in overseeing all clinical support staffing in the ambulatory clinical setting; delegates responsibilities appropriately and within scope.
  • Identifies high risk patients and works with the appropriate services and interdisciplinary team members to achieve the best possible patient care and outcomes.
  • Promotes shared decision making with patients in regards to healthcare; educates the patient and/or family regarding the patient's clinical condition, treatment, and their role in their healthcare decisions.
  • Utilizes a variety of telecommunications technologies to assess, triage, provide nursing consultation, and perform follow-up and surveillance of patient status and outcomes.
  • Collaborates and actively engages with the care team and practice leadership to identify opportunities and solutions to improve quality, patient access and cost savings.
  • Demonstrates compliance with regulatory requirements i.e. MIPS, fall assessment.
  • Participates in satellite quality and safety initiatives as indicated.
