Activity Assistant - Leisure Programs

Ebenezer-Lake City Care Center TCU LTC offers a comprehensive benefits package and competitive wages ranging from $16.04 - 17.89 / hour.

We are seeking an Activity Assistant to support our adult residents in our Skilled Nursing Facility in Lake City, MN This role assists with the implementation of a quality leisure/social events program designed to help residents maintain their personal potential both physically, socially, and mentally. This position will also drive the company van/bus for resident outings.

This patient support schedule includes;

  • 64 hours in two weeks
  • Day shift
  • Monday - Friday; weekends as needed

Key Responsibilities:

  • Assist with leisure programs and activities
  • Lead social events and activity programs
  • Communicate and coordinate activity/leisure programs with all departments
  • Gather and transport residents for programs and activities
  • Assist other departments with special events and family functions
  • Document as required
  • Offer suggestions and ideas to improve the events program to the Director of Active Living
  • Assist in meeting the psychosocial needs of the residents
  • Encourage residents participation
  • Report change in resident attitude, mood or physical status to appropriate staff
  • Promote residents independence by offering choices and fostering self-help skills
  • Assist residents in developing relationships and effectively socializing
  • Assist with virtual visits upon recommendations of Active Living Coordinators and Active Living Director
  • Assist in implementing pet visits based on recommendations of Active Living Coordinators and Active Living Director
  • Assist Active Living Coordinators in maintaining compliance with state and federal regulations regarding medical record documentation and participation
  • Assist in completing MDS, Assessments, Care Plans, and Progress notes for Active Living
  • Complete and update participation records
  • Gather information from other team members as it relates to residents' needs
  • Drive company van/bus for resident outings
  • Review and complete Vehicle Requirements - Transporting Residents Checklist prior to each resident outing
  • Maintain a log of transportation services and tasks performed throughout the driving period
  • Responsible for the safety and welfare of passengers
  • Practice sound, safe judgement when performing transportation services including but not limited to: fire/disaster procedures and infection control procedures
  • Recognize and act appropriately on safety hazards and incidents
  • Follow all laws regarding operating a motor vehicle
  • Immediately report any traffic violations or accidents
  • Other duties as assigned and directed
  • Respond to resident and building emergencies per building policies and procedures
  • Adhere to applicable rules and regulations
  • Take photos of new admits
  • Assist with transportation of residents to and from dining areas
  • Perform environmental and other responsibilities as assigned (order and cleanliness)

Requirements:

  • 0-1 years of experience
  • Current Driver's License (in good standing)
  • Working knowledge of basic Microsoft applications

Preferred:

  • Associate Degree - Therapeutic Recreation, Music Therapy or related field
