As a Physical Therapist, you will work independently in the community as part of an interdisciplinary care team, conducting comprehensive assessments of patients and developing personalized rehabilitation therapy plans.

Key Responsibilities:

  • Practice independently in the community, conducting comprehensive assessments of patients and developing personalized rehabilitation therapy plans.
  • Deliver high-quality rehabilitation therapy to patients in their homes or care facilities, constantly evaluating evolving patient needs and responding with plan of care adjustments.
  • Collaborate with interdisciplinary care teams to ensure seamless care delivery and patient outcomes.

Requirements:

  • Minimum of one year of physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern.
  • Licensure to practice as a Physical Therapist in New York State.
  • Valid driver's license required for all areas outside the borough of Manhattan.
