About the Role

Atrium Health is seeking a highly skilled Licensed Physical Therapy Assistant Care Coordinator to join our home health team. As a licensed professional, you will be responsible for coordinating patient care and developing individualized treatment plans to ensure optimal outcomes.

Key Responsibilities

  • Assess patients' physical abilities and develop personalized treatment plans.
  • Coordinate with other healthcare professionals to ensure comprehensive patient care and coordinate efforts to achieve optimal outcomes.
  • Maintain accurate records of patient progress, including notes, evaluations, and treatment plans.

Requirements

  • Degree from an accredited Physical Therapist Assistant program.
  • Licensure as a Physical Therapist Assistant in the state where you will work.
  • 1-2 years of experience in home health or clinical settings preferred.
  • BLS (Basic Life Support) certification required.
