Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Develop and implement individualized plans of care to meet the unique needs of each patient.
  • Conduct OASIS assessments to identify patient needs and develop effective treatment plans.
  • Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and outcomes in accordance with agency policies and regulations.
Requirements:
  • Current licensure as a Physical Therapist in the state of employment.
  • Valid driver's license and auto insurance.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to lift and move patients as needed.
