Job Title: Home Health Physical Therapist

We are seeking a skilled and compassionate Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a Home Health Physical Therapist, you will play a vital role in providing high-quality patient care and improving the lives of our patients.

Job Summary:

The successful candidate will be responsible for providing skilled physical therapy services to patients in their homes, as ordered by the attending physician. This will include assessing patients' needs, developing individualized plans of care, and implementing treatment plans to achieve optimal patient outcomes.

Key Responsibilities:
  • Plan of Care: Develop and implement individualized plans of care for patients, including assessing their needs, setting goals, and evaluating progress.
  • Patient Outcomes: Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as exercise, massage, heat, water, etc.
  • Minimize Patient Risk: Help decrease re-hospitalizations by front-loading visits for high-risk patients and teaching them preventative measures and making good decisions.
  • Family Educator/Advocate: Counsel patients and their families on the disease/injury and how to manage in all phases of physical therapy. Instruct patients and family members on proper use of equipment, such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Supervision: Oversee and supervise therapy services provided by PTAs and, when appropriate, nurse assistants.
  • Communication/Collaboration: Communicate with PTAs and nurse aides on supervisory visit results.
  • Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
  • Operations: Attend in-service training and mandatory agency meetings.
Requirements:
  • Physical Therapist: Current license in the state of employment.
  • Valid Driver's License: Valid driver's license and auto insurance in your name as a driver.
  • Capable of All Physical Demands: Capable of all physical demands.
