Position Overview

St. Francis Children's Center is seeking a dedicated and fully bilingual Service Coordinator proficient in both Spanish and English to join our Children's Long Term Support Waiver Program.

Role Responsibilities

This role encompasses various essential duties, including:

  • Conducting regular home visits to children with special needs.
  • Developing and overseeing each child's Individual Service Plan.
  • Facilitating coordination of community services for families.
Work Environment

Our Service Coordinators have the flexibility to work remotely, although in-person client visits and team meetings are necessary.

Qualifications

Ideal candidates will meet the following criteria:

  • A Bachelor’s degree in Social Work or a related discipline.
  • A minimum of one year of professional experience with children who have special needs.
  • Fluency in reading, writing, and speaking Spanish.
  • Prior experience in case management is highly desirable.
Skills and Attributes

The successful candidate will demonstrate:

  • Exceptional communication abilities.
  • Flexibility in scheduling, with limited evening work as needed.
  • Meticulous attention to detail.

Reliable and insured transportation is required for home visits and meetings.
