Job Summary

We are seeking a skilled and compassionate Nurse Resident/Home Health to join our team at SSM Health. As a key member of our healthcare team, you will provide high-quality nursing care to residents in a home health setting.

Responsibilities
  • Provide skilled nursing care to residents according to established care plans and federal/state regulations.
  • Administer medications and treatments according to standards of care and maintain competency in all required nursing skills.
  • Provide guidance and direction to ancillary staff and monitor the workflow of the unit to ensure coordination of care.
  • Use the nursing process for assessment, nursing diagnosis, planning, intervention, and evaluation, and report significant changes to physicians as indicated.
  • Ensure accurate inventory of medications at the beginning and end of each shift and distribute medications to residents according to the 5 rights.
  • Communicate with residents, guardians, and family members on the current condition and needs of residents in accordance with regulations and standards of practice.
  • Provide teaching and training to residents and families as needed and ensure that preventative measures are in place, such as specialty mattresses, foam boots, and skin checks.
  • Follow up on reports of rashes, open areas, edema, blisters, skin tears, or bleeding and respond promptly to alarms.
  • Ensure that restraint alternatives are in place, including personal alarms, cushion alarms, and wheelchair belts.
  • Provide quality end-of-life care, using comfort baskets to meet resident and family needs, and ensure hygiene, comfort, and spiritual needs are met.
  • Perform other duties as assigned.
Requirements
  • Graduate of an accredited school of nursing.
  • No experience required.
