**Job Title:** Hospice Support Specialist
Affinity Care of Ohio is seeking a skilled and compassionate hospice support specialist to join our team. In this vital role, you will play a crucial part in delivering high-quality patient care and supporting the needs of patients and their families during a challenging time.
**Estimated Salary:** $40,000 - $50,000 per year
The successful candidate will be an excellent communicator, empathetic listener, and able to work collaboratively with the hospice care team. Key Responsibilities:
- Perform various tasks as assigned by the registered nurse, including personal care, light housekeeping, and laundry assistance
- Provide emotional support and companionship to patients and their families
- Document patient care and progress accurately and thoroughly
- Collaborate with the hospice care team to ensure seamless delivery of services
- Maintain confidentiality and adhere to HIPAA guidelines
Essential Qualifications:
- Completion of a state-approved CNA course or equivalent training
- Current Healthcare Provider CPR certification
- At least 18 years old; valid driver's license and reliable transportation
- Basic computer skills and proficiency in EMR
