Job DescriptionExcelin Health Services is seeking a highly skilled and experienced Clinical Manager/Director of Patient Care Services to lead our hospice services team. The ideal candidate will have a strong background in healthcare leadership and management, with a focus on patient-centered care and quality improvement.

Job Summary:

The Clinical Manager/Director of Patient Care Services is responsible for the overall direction and management of hospice services, ensuring that patients receive high-quality, compassionate care that meets their unique needs and preferences.

Key Responsibilities:

  1. Develop and implement effective strategies to improve patient outcomes, satisfaction, and quality of care.
  2. Lead and manage a team of clinical staff, providing guidance, support, and coaching to ensure that they have the skills and resources needed to deliver exceptional care.
  3. Collaborate with other departments and stakeholders to ensure seamless coordination of care and services.
  4. Monitor and evaluate the performance of clinical staff, identifying areas for improvement and implementing changes as needed.
  5. Stay up-to-date with industry trends, best practices, and regulatory requirements, ensuring that our hospice services remain compliant and effective.

Requirements:

  1. Registered Nurse with current/active California license in good standing with the BRN.
  2. Bachelor's degree in Nursing from an accredited program by the National League for Nursing. Master's degree preferred.
  3. At least three years of experience in healthcare leadership and management, with a focus on hospice care.
  4. Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  5. Ability to analyze data, identify trends, and make informed decisions to improve patient outcomes and quality of care.
