Clinical Pharmacist for Medication Management

Job Overview

The Clinical Pharmacist specializing in Medication Therapy Management (MTM) is dedicated to delivering tailored and patient-focused MTM services aimed at enhancing medication therapy to achieve optimal clinical, economic, and humanistic results.


Key Responsibilities

The MTM Clinical Pharmacist engages in a variety of services, including:

  • Comprehensive medication assessments
  • Targeted intervention strategies
  • Monitoring adherence to prescribed therapies
  • Cost-effective medication solutions
  • Facilitating transitions in care
  • Conducting post-discharge medication reviews

The pharmacist is also responsible for educating patients on appropriate medication usage, potential adverse effects, and necessary lifestyle adjustments. Additionally, the pharmacist will monitor patients' responses to their medication regimens and provide recommendations to both patients and healthcare providers.


Collaboration and Documentation

Accurate documentation of all professional activities is essential. Consultations may occur in person or through telephonic and virtual platforms. The pharmacist will work collaboratively with interdisciplinary teams across the healthcare system and community, under the guidance of leadership and collaborating physicians, to manage the medication therapy of referred patients.


Qualifications

The ideal candidate should possess extensive pharmacotherapy knowledge to address patient medication needs effectively. Strong oral communication skills, along with demonstrated customer service abilities, are essential. Attention to detail, critical thinking, and problem-solving skills are required. The pharmacist will also serve as a mentor and preceptor for pharmacy students.


Education and Experience

A graduate of an ACPE accredited pharmacy program with an advanced degree (e.g., Pharm.D) is required. A minimum of 7 years of relevant experience in an ambulatory or community pharmacy setting is preferred, along with a solid understanding of MTM billing and experience in medication transitions of care. Completion of a PGY 1 Pharmacy Practice residency and a PGY 2 residency in a relevant specialty is advantageous.

Valid and active pharmacy licensure in both Tennessee and Virginia is necessary. Board certification in a pharmaceutical specialty (e.g., BCPS, BCACP, BCGP) is required within two years, and MTM certification is preferred.
