Key Responsibilities
  • Anticoagulation Clinic Oversight: Provide guidance and oversight for the Anticoagulation Clinic Service, ensuring seamless delivery of care to patients.
  • Therapy Review and Adjustment: Perform regular review of drug regimens and laboratory data to adjust anticoagulation therapy as needed, maximizing outcomes for patients.
  • Patient Monitoring and Education: Conduct regular patient monitoring to ensure desired outcomes are met, providing education and support to patients and families regarding anticoagulation therapy.
  • Documentation and Communication: Maintain accurate and organized documentation of recommendations, interventions, and outcomes in Coagclinic and Webcare, communicating effectively with prescribers and other healthcare professionals.
  • Competency and Advocacy: Maintain competency in anticoagulation care, serving as an active patient advocate and using a team approach to working with staff and others.
Requirements
  • Education and Licensure: At a minimum, a BS in Pharmacy from an accredited college of pharmacy, with PharmD preferred. Iowa Pharmacist Licensure, either by examination or by reciprocity.
  • Experience and Skills: Demonstrated competence in providing direct patient care, with previous anticoagulation management experience preferred. Self-motivated, detail-oriented individual able to work with minimum supervision, with good communication and leadership skills.
