Position Overview:
The Clinical Pharmacist in the Anticoagulation Clinic plays a vital role in managing patient care related to anticoagulation therapy. This position involves collaborating with healthcare providers to ensure optimal therapeutic outcomes for patients receiving anticoagulant medications.
Key Responsibilities:
- Oversee and direct the Anticoagulation Clinic operations.
- Conduct thorough reviews of medication regimens and laboratory results, making necessary adjustments to anticoagulation therapy to enhance patient outcomes.
- Monitor patients diligently to ensure compliance with established protocols and clinical guidelines.
- Provide education to patients and their families regarding anticoagulation treatment plans.
- Maintain accurate and organized records of clinical recommendations, interventions, follow-ups, and outcomes.
- Facilitate effective communication with healthcare providers who refer patients to the clinic.
- Uphold a high level of competency in anticoagulation management and advocate for patients' best interests.
Work Schedule:
PRN; as needed.
Qualifications:
- A minimum of a Bachelor of Science in Pharmacy from an accredited institution; Doctor of Pharmacy (PharmD) preferred.
- Valid Iowa Pharmacist License, obtained through examination or reciprocity.
- Proven experience in delivering direct patient care.
- Previous experience in anticoagulation management is preferred.
- Self-driven and detail-oriented with the ability to work independently.
- Strong communication and leadership skills.
- Completion of Mandatory Reporting: Child and Dependent Adult Abuse training within 6 months of hire, with renewal every 5 years.
