Job Summary

The University of Iowa Health Care is seeking a highly skilled Clinical Pharmacist to join our Internal Medicine team. As a key member of our healthcare team, you will be responsible for providing comprehensive pharmaceutical care to adult patients in our acute care setting.

Key Responsibilities
  • Provide therapeutic drug monitoring and medication reconciliation services to ensure accurate and safe medication use.
  • Collaborate with interdisciplinary teams to develop and implement patient-centered care plans.
  • Conduct medication histories and reconcile home medications with inpatient orders.
  • Communicate with prescribers and other healthcare professionals to ensure optimal patient care.
  • Document patient care activities and medication errors in accordance with departmental policies.
  • Participate in quality improvement initiatives and contribute to the development of best practices in pharmaceutical care.
Requirements
  • PharmD degree or Bachelor of Science in Pharmacy.
  • Current licensure by the Iowa Board of Pharmacy Examiners.
  • One year of applicable pharmacy experience, including time spent completing an ASHP-accredited pharmacy residency.
  • Excellent written and verbal communication skills.
  • Job-related experience with and/or commitment to service excellence.
Preferred Qualifications
  • Experience in a hospital pharmacy setting.
  • Basic Life Support (BLS) certification or eligibility for completing BLS certification.
  • Satisfactory completion of an ASHP-accredited PGY1 residency in pharmacy.
  • Board certification in Pharmacotherapy or another specialized field.
