- Monitor and record adverse drug reactions, adherence gaps, and barriers to patient adherence.
- Collaborate with healthcare professionals to address medication-related concerns and provide education on safe medication use.
- Document all clinical activities and interventions in the electronic record.
- Serve as a consultation resource for medical professionals and answer inquiries on drug therapy and products.
- Consult with patients and caregivers on medication therapy and provide education on proper drug administration.
- Assist in discharge planning and counsel patients and caregivers on medication-related topics.
- Contribute to the development and maintenance of medication-related guidelines and protocols.
- Detect and report medication errors and near misses.
- Ensure readiness for regulatory and accreditation surveys.
- Participate in quality improvement and medication use review activities.
- Prepare and deliver presentations to facilitate learning and disseminate best practices.
- Utilize a solution-based approach to enhance patient satisfaction and care.
- Graduate of an ACPE-accredited School of Pharmacy with a Bachelor's of Science in Pharmacy.
- Doctor of Pharmacy (Pharm D) degree preferred.
- Completion of a post-graduate residency and/or board certification preferred.
- Ability to work independently with minimal direct supervision and adapt to changes in workload.
- Strong written and verbal communication skills.
- Current licensure to practice in the state of NH and willingness to seek license reciprocity in multiple states if required.
