Nurse Coordinator

Eliot Community Human Services is seeking an experienced Registered Nurse to coordinate residential services for adults with developmental disabilities and brain injuries. The successful candidate will have 3 years of related experience and the ability to establish supportive relationships with individuals with developmental disabilities and brain injuries.
Responsibilities- Provide education and consultation to individuals and staff on medical issues, treatments, medications, wellness, and prevention.
- Provide oversight, guidance, and consultation in all residential programs around medical protocols, medication management, and coordination of care.
- Review all medical documentation, complete medical assessments, and prepare medical documentation for all new referrals and discharges.
- Perform direct nursing care and medical support services to all residential programs as needed.
- Hire, train, and supervise nursing staff for the Brain Injury programs.
- Meet with managers and nursing staff when an individual receives any new medical diagnosis, is prescribed a new medication, diet, medical device, or protocol.
- Participate in the development of treatment goals for each individual related to medical, medication, or health concerns.
- Develop working relationships with community medical providers and VNA-related services to assist with coordinating medical care for individuals.
Schedule: Full-time, M-F, 8am-4pm or 9am-5pm
Salary: RN $85,000.00
