Nurse Care Coordinator

Job Summary:
We are seeking a highly skilled Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will be responsible for providing comprehensive patient care through the nursing process, delivering safe and therapeutic care in accordance with established standards, policies, and procedures.
Key Responsibilities:
- Assess patient needs and develop individualized plans of care
- Implement and evaluate the effectiveness of care plans
- Collaborate with healthcare team members to coordinate patient-centered care
- Anticipate and respond to changes in patient condition
- Provide education and support to patients and families
Requirements:
- Registered Nurse license issued by the state in which you practice
- Basic Life Support (BLS) certification
- 1 year of experience in clinical nursing
