About the Role

Fellowship Square is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Home Health Team. As a key member of our team, you will play a vital role in providing high-quality care to our senior residents, ensuring their dignity and quality of life are preserved.

Responsibilities
  • Provide skilled nursing services to Home Health patients under the direction of the RN Case Manager or Director of Nursing.
  • Follow physician orders and HHA policy and procedures.
  • Ensure compliance with the State of AZ scope of Practice and Nurse Practice act.
  • Complete mandatory CEU's to maintain LPN licensure.
  • Participate in formal continuing education sessions and in-service sessions.
  • Maintain open relationships and communication with clients, clients' families, physicians, and other HHA staff involved in the care of the client.
  • Follow all State and Federal rules and regulations as they pertain to the Home Health Center.
  • Document each home health visit within the medical record to include significant observations regarding the resident's needs for and/or response to medications/treatment.
Qualifications
  • Must be a graduate of an approved school for licensed practical nursing or registered nursing.
  • Must hold a current, valid LPN license in the State of Arizona.
  • Minimum of 2 years' experience.
  • Must have an understanding of DHS rules.
  • Previous experience in a geriatric setting preferred.
  • Must be flexible and have a desire to communicate and work with seniors.
  • Knowledge of Home Health rules and regulations required.
