About the RoleWe're seeking a skilled and compassionate Nurse Practitioner to conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. As an independent contractor, you'll have the flexibility to choose your own schedule and work at your own pace.
Key Responsibilities- Conduct in-home visits to assess patients' overall health and well-being

- Work collaboratively with patients, families, and healthcare teams to ensure seamless care coordination
