Nurse Practitioner for In-Home Visits

About the RoleWe're seeking board-certified Nurse Practitioners to join our team of healthcare professionals who share our commitment to delivering exceptional patient care. As an In-Home Health Evaluation Specialist, you'll conduct comprehensive assessments in patients' homes, focusing on their overall health and well-being. Your expertise will help identify potential barriers to care and inform personalized treatment plans.
Key Responsibilities- Conduct in-home health evaluations, including medication and medical history reviews, and physical evaluations
- Develop and implement personalized treatment plans in collaboration with patients and their healthcare teams
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
- Stay up-to-date with the latest clinical guidelines and best practices in geriatric care
- A competitive hourly rate of $80.00, with opportunities to increase earnings through our Market Multiplier program
- A flexible schedule, with shifts available Monday through Sunday
- Short-term or long-term contract options, allowing you to choose the arrangement that suits your needs
- Malpractice coverage and independent contractor status
- A comprehensive benefits package, including medical, dental, and vision coverage
- Master's degree in Nursing (MSN) or a related field
- Current Minnesota Nurse Practitioner License (or equivalent in other states)
- Ability to relocate to Grand Rapids, MN (required for some positions)
- Willingness to travel up to 50% of the time (preferred)
