The Virtual Registered Nurse (RN) is a key member of the Nursing Care Team at Trinity Health Corporation, responsible for collaboration, communication, and facilitation of optimal provision of care through a computerized and digital platform.
As a Virtual RN, you will be responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of the patient's plan of care from admission to discharge.
Key Responsibilities- Assess patient needs and develop individualized care plans to ensure seamless transitions of care.
- Utilize clinical knowledge, critical thinking skills, and the principles of case management and adult education to coordinate, implement, and support the plan of care.
- Monitor quality metrics specific to the department and ensure complete and accurate documentation in the patient record.
- Perform the roles of mentor, clinician, educator, consultant/collaborator, and navigator to ensure effective care coordination.
- Graduate from an accredited professional school of nursing with valid RN licensure in the State of Ohio.
- Minimum of eight (8) years of experience in health condition management, including adult condition management, with Electronic Medical Record (EMR) experience.
- Baccalaureate Degree in the Science of Nursing (BSN) degree from an accredited school of nursing preferred.
- Specialty credentialing and educational degree according to clinical nursing practice specialty area preferred.
