Job Summary:
UnitedHealth Group is seeking a skilled RN Field Clinical Care Coordinator to join our team. As a key member of our Integrated Care Model, you will play a vital role in addressing the medical needs and social determinants of health for our members.
Key Responsibilities:
- Assess, plan, and implement care strategies for members that are individualized by patient and directed toward the most appropriate, least restrictive level of care.
- Manage the care plan throughout the continuum of care as a single point of contact.
- Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members.
- Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the healthcare team.
- Expect to spend about 50-80% of your time in the field visiting our members in their homes or in long-term care facilities.
Requirements:
- Current, unrestricted RN license in MA plus 2+ years of clinical work experience.
- 2+ years of experience in long-term care, home health, hospice, community health, or assisted living.
- 2+ years of experience working with MS Word (create and edit) and Outlook (email and calendar).
- Ability to travel in assigned region to visit members in their homes and/or other settings, including community centers, hospitals, or providers' offices.
- Access to high-speed internet at residence.
Preferred Qualifications:
- BSN Degree or working toward a BSN.
- Experience with electronic charting.
- Field-based work experience.
- Background in managing populations with complex medical or behavioral needs.
- Bilingual.
