Key Responsibilities

The Home Health Patient Care Manager oversees and coordinates clinical services, directing nursing care to patients in their homes as prescribed by physicians and in accordance with relevant laws, regulations, and agency policies.
  • Delivers clinical services within the scope of nursing practice as defined by state laws, following the care plan and collaborating with the healthcare team.
  • Handles referrals, assigns clinicians appropriately, evaluates patient orders, and schedules initial care visits.
  • Coordinates patient home health benefits, assesses medical necessity, and manages ongoing insurance approvals.
  • Continuously assesses patient needs, ensuring individualized care that meets eligibility criteria and adheres to physician orders.
  • Reviews daily assessments and care plans, consulting with clinicians for recommendations as necessary.
Qualifications

Education & Experience
  • Valid RN licensure in the state of practice
  • Current CPR certification is mandatory
  • Possession of a valid Driver's License, vehicle insurance, and access to reliable transportation
