Job Summary

The registered nurse care coordinator is an integral part of the healthcare team, responsible for coordinating care for patients across multiple settings, including inpatient, outpatient, and home health. This role requires a clinical expert who can work collaboratively with other healthcare professionals to assess patient needs, develop a plan of care, and optimize healthcare outcomes.

Key Responsibilities
  • Collaborate with providers, case managers, and other healthcare professionals to assess patient needs and develop a plan of care
  • Provide triage support to patients across multiple clinical areas
  • Monitor patient outcomes and document patient response to care
  • Ensure providers are informed of outcomes
  • Serve as the point of contact for patients and the care team to manage complex health needs
  • Provide patient consultations and information on recommended treatments
  • Coordinate prescription refill management, including creation and submission of prior authorization forms
  • Complete clinical paperwork, including lab, school, and leave forms, medical releases, emergency, and travel letters, durable medical equipment, etc.
  • Assist in coordinating complex patient visits
  • Support patients through pre-visit clinical assessments, including collection of appropriate history
  • Participate in Quality Assurance/Performance Improvement (QAPI) activities as requested
Requirements
  • Current or compact RN licensure in the state of North Carolina required
  • 3 years of nursing experience required
  • BSN required
