The Role
Pine Park Health is in search of a Primary Care Provider - Nurse Practitioner to deliver care at Friendship Village. We are looking for an individual who is enthusiastic about managing a smaller panel of patients with complex health needs, contributing to a value-driven care framework, and collaborating with a dedicated team to provide exceptional patient services. This model emphasizes personalized care and leverages our primary care coordinators to optimize provider efficiency.
Key Responsibilities
- Deliver on-site primary care services to older adults at Friendship Village.
- Design and implement comprehensive, patient-centered care plans for all primary care patients.
- Collaborate with the Medical Director to establish and refine policies addressing daily practice challenges, cultural dynamics, and clinical guidelines, particularly for emerging health issues.
- Facilitate regular team meetings and provide leadership to the clinical staff.
- Identify collaborative opportunities with senior living management and care teams to enhance the execution of Pine Park care plans and improve patient outcomes.
Qualifications
- Certified Nurse Practitioner with a preference for experience in geriatrics.
- Adaptable to a dynamic and fast-paced work environment.
- Willingness to provide clinical care on-site.
- Commitment to working with patients with complex health needs and contributing to a holistic care approach.
- Strong collaborative spirit and appreciation for input from all clinical team members.
- Excellent written and verbal communication skills.
