The Role
Pine Park Health is currently seeking a Primary Care Provider - Nurse Practitioner to deliver care at our partnered communities. We are looking for a dedicated professional eager to manage a focused patient panel, engage in a value-driven care approach, and collaborate with a passionate team to ensure exceptional patient outcomes. This role emphasizes a patient-centered care model that optimizes the contributions of our primary care coordinators.
Key Responsibilities
- Deliver onsite primary care services to older adults.
