The Role

Pine Park Health is in search of a Primary Care Provider - Nurse Practitioner to deliver care at a senior living community. We are looking for an individual eager to manage a smaller panel of patients with complex health needs, contribute to a value-driven care approach, and collaborate with a dedicated team to ensure exceptional patient outcomes. This model emphasizes personalized care, utilizing our primary care coordinators to optimize provider efficiency.

Key Responsibilities

  • Deliver onsite primary care services to elderly residents.
  • Design and implement comprehensive, individualized care plans for all patients.
  • Collaborate with the Medical Director to formulate policies addressing clinical practices and emerging health issues.
  • Facilitate regular team meetings and provide guidance to the clinical staff.

Qualifications

  • Certified Nurse Practitioner with a preference for experience in geriatrics.
