Position Overview:
Clinical & Support Options is seeking a dedicated Supervisor for our Low Threshold Housing (LTH) initiative. This role is essential in providing both administrative and programmatic oversight for the Friends of the Homeless (FOH) Low Threshold Housing program, ensuring effective collaboration of care within the organization and with property stakeholders.
Key Responsibilities:
As an LTH Supervisor, you will:
- Offer leadership and guidance to case management personnel, ensuring comprehensive program oversight, including on-call support for urgent needs.
- Directly oversee Housing Case Managers, facilitating training, managing job descriptions, distributing caseloads, and providing constructive feedback.
- Conduct regular one-on-one meetings with Housing Case Managers to review caseloads and provide supervision.
- Organize and lead regular staff meetings, ensuring adequate coverage as necessary.
- Administer probationary and annual performance evaluations for staff.
- Ensure timely updates from Housing Case Managers regarding program participants' move-in dates, relocations, or terminations.
- Act as the primary contact for landlords, property managers, and referral sources associated with the FOH Low Threshold Program.
- Assist in coordinating move-ins, including scheduling, key distribution, and furniture delivery.
- Collaborate with the Director to oversee all operational aspects of the program, ensuring client needs are prioritized.
- Maintain accurate client records in relevant systems and ensure compliance with third-party requirements.
- Update and manage comprehensive tenant and staff information within the agency's electronic health record system.
Qualifications:
To be successful in this role, candidates should possess:
- A Bachelor's degree (preferred).
- Previous supervisory experience.
- Experience working with homeless or at-risk populations (preferred).
- Proficiency in computer applications.
- A valid driver's license and reliable transportation.
