Job Description

Crowne Cove is seeking a highly skilled Resident Care Coordinator to join our team. As a key member of our Assisted Living Department and/or Generations/Memory Care unit, you will play a vital role in ensuring the optimal care and well-being of our residents.

Responsibilities:
  • Coordinate all care services provided to residents, ensuring compliance with Federal and State policies governing resident care.
  • Supervise and develop staff within the Assisted Living Department and/or Generations/Memory Care unit, focusing on qualified candidates with experience in care and management of individuals with dementia.
  • Coordinate departmental schedules, ensure adequate staffing, and oversee continuing education and staff development programs for caregivers and medication aides.
  • Conduct or participate in orientation for new staff members, ensuring they are knowledgeable of department goals, policies, and procedures.
  • Coordinate medication provision, emergency procedures, record keeping, and resident records, ensuring all care staff complete state-required training.
  • Monitor and assist with personal care services, medication provision, and documentation, ensuring all necessary records are completed.
  • Arrange for coverage of job duties during employee rest periods, meal periods, and/or absences, either through delegation or personal completion of duties.
  • Assist with the completion of resident assessment programs, administer pre-admission and periodic assessments, and monitor changes in resident status.
  • Conduct interviews and observations to alert the Director of Resident Care and others of changes in resident physical or emotional health.
  • Complete all required data entry into Eldermark, ensuring maintenance of resident care information.
  • Develop methods for coordination of nursing services with other resident services to ensure continuity of care.
  • Act as care manager and resource person for residents and their families, contacting physicians, family, and others as needed.
  • Coordinate prescription orders with doctors' offices and pharmacy delivery of medications, supervising central storage and delivery of medications.
  • Assist with monitoring infection control programs, recognizing patterns of in-house infections and their potential spread.
  • Conduct property tours and promote the property to members of the community, supporting administration in referral development and maintaining high census.
  • Participate in property surveys (inspections) made by authorized governmental agencies, maintaining a safe and secure environment for all staff, residents, and guests.
Qualifications:
  • High school diploma or equivalent.
  • Certifications as required by the state.
  • Prefer one (1) year experience providing residential care to the elderly or similar experience with specialized dementia care programs.
  • Prefer six months supervisory experience in a health-related field.
