Position Overview

The Head of Social Services is tasked with overseeing the comprehensive management of the psychosocial needs of residents at Windsor Health & Rehabilitation Center LLC. This role requires the ability to provide counseling or coordinate with psychiatric professionals to enhance patient well-being and progress. The individual must demonstrate both independence and the capacity to collaborate effectively within an interdisciplinary team.

Responsibilities

The authority vested in this position includes:

  • Evaluating policies and procedures to ensure social services for residents align with federal, state, and local regulations.
  • Conducting interviews, assessments, and crafting personalized plans to address residents' needs.
  • Reviewing the facility's social service program and offering insights to the administration.
Key Duties

Responsibilities encompass, but are not limited to:

  1. Performing admission assessments for new residents.
  2. Conducting interviews with incoming patients.
  3. Completing necessary MDS sections.
  4. Finalizing ASCEND documentation.
  5. Developing care plans as required.
  6. Providing ongoing support to residents, including conducting 72-hour meetings and maintaining care notes.
  7. Investigating allegations of mistreatment and grievances thoroughly.
  8. Facilitating discharge planning to ensure appropriate living arrangements are secured prior to patient release.
  9. Addressing financial concerns through discussions with patients and their families.
  10. Intervening in family crises on behalf of patients.
  11. Offering counseling for significant life events impacting residents.
  12. Submitting ASCEND documentation to relevant agencies upon admission.
  13. Assisting residents in obtaining level of care extensions.
  14. Collaborating with the Business Office Manager to support long-term care residents in securing benefit approvals for extended stays.
Qualifications

Ideal candidates will possess:

  • A Master’s degree in Social Work (MSW).
  • A minimum of 2 years of experience in a Long-Term Care environment.
  • In the absence of a Master’s degree, a Bachelor’s degree in Social Work with 5 years of relevant experience.
  • Flexibility in scheduling.
  • Strong communication skills, with the ability to address complaints and seek resolutions effectively.
