Job Title: Director of Health and Wellness

Bell Oaks Place is seeking a highly skilled and dedicated Director of Health and Wellness to join our team.

Responsibilities:

  • Ensure compliance with federal, state, and local laws and regulations related to clinical care.
  • Analyze occurrence trends and develop/implement improvement plans as needed.
  • Collaborate with the Administrator and other team members to maintain resident and team member health and safety.
  • Partner with the pharmacy consultant to provide optimal pharmaceutical services to residents.
  • Provide clinical expertise to licensed nurses.
  • Identify ongoing needs and services of residents through the assessment/Service Plan process.
  • Ensure resident service plans are updated as required by state regulations.
  • Maintain communication with residents' families and/or responsible parties regarding changes in care or health concerns.
  • Schedule clinical staff on a monthly basis.
  • Ensure adequate clinical supplies are available.
  • Participate in a rotating on-call schedule.

Qualifications:

  • Current state license as a Licensed Practical Nurse or Registered Nurse.
  • Minimum 1-year supervisory experience.
  • Minimum 2 years geriatric experience.
  • Prior assisted living, home health, or long-term care experience preferred.
  • Competence in assessment skills, injections/medication administration, follow-up, and triage.
  • Leadership skills, including confidence, motivation, and inspiration of others.
