POSITION SUMMARY:

The Independent Living Program Case Coordinator plays a vital role within a therapeutic team, delivering essential support to individuals enrolled in a Transitional Living Program. This role is designed to foster a therapeutic atmosphere that encourages healing, wellness, and optimism. The Case Coordinator will provide guidance and mentorship to individuals, assisting them in successfully integrating into the program while promoting the development of communication, relationship, and interpersonal skills.

KEY RESPONSIBILITIES:

1. Program Coordination:
The Case Coordinator will engage in the communication and coordination of program activities, including:

  • Monitoring facility logs and addressing operational matters with staff.
  • Overseeing staff during daily activities.
  • Reviewing non-case specific correspondence.
  • Completing communication logs and attending all relevant staff meetings.
2. Facility Maintenance:
The Case Coordinator will ensure the upkeep of the Transitional Living Program facilities by:
  • Supervising program activities and addressing operational issues.
  • Filing maintenance requests and completing necessary reports.
3. Reporting:
Weekly reports will be submitted to the Director of Independent Living Programs, detailing resident concerns, property issues, and scheduling for the upcoming week.

4. Compliance:
The Case Coordinator is responsible for adhering to all case management requirements, which include:

  • Conducting individual or group meetings to discuss licensing and certification processes.
  • Maintaining accurate case records and documentation.
5. Training:
All training requirements must be completed and maintained in a timely manner, including:
  • Supervision and participation in relevant meetings.
  • Crisis intervention training.
6. Crisis Support:
The Case Coordinator will assist in managing crisis situations and participate in an on-call support rotation, ensuring the safety and well-being of all residents.

7. Resident Care:
The Case Coordinator will prioritize the care and welfare of each youth, which includes:

  • Daily supervision and mentoring of residents.
  • Administering medications and first aid as needed.
  • Documenting daily activities and incidents.
8. Communication:
Regular communication with families and other professionals involved in the youth's care is essential for effective case management.

QUALIFICATIONS:

  • A minimum of three years of experience in case management or a related field.
  • CPR and First Aid Certification (training provided).
  • Preferred: Five years of experience working with youth in residential settings.
  • Bachelor's Degree in a Human Services field is desirable.
WORKING CONDITIONS:
This position may involve exposure to various environmental factors and requires the ability to work a flexible schedule, including weekends.
