Patient Care Coordinator

We are seeking a highly skilled and compassionate Patient Care Navigator to join our team at MyMichigan Health. As a Patient Care Navigator, you will play a critical role in supporting patients and professional staff in improving care access, care transitions, and coordination of patient care resources.
Key Responsibilities- Identify and Address Social Barriers: Utilize motivational interviewing techniques to identify social risk factors that may be hindering patients from meeting their healthcare goals.
- Link Patients to Resources: Complete required clinical screenings to assist with identification of patients' risk factors and link them to community and medical resources needed to address social determinants of care and/or barriers to healthcare goals.
- Coordinate Care: Coordinate medical appointments, assist patients in addressing their barriers to medication regimen adherence, actively work with patients to increase adherence to scheduled appointments, and coordinate follow-up with non-clinical services such as transportation, home health, homeless shelters, home meal delivery, etc.
- Document Patient Interactions: Maintain appropriate documentation of patient contact, referrals made, and services provided.
- Communicate Effectively: Effectively and professionally complete the appropriate workflow and documentation for all assigned patients and communicate all urgent concerns regarding patient care needs to appropriate care team members in a timely manner.
- Participate in Process Improvement: Participate in process improvement projects and specific clinical tasks as assigned to improve patient access.
- Education: High School Diploma or GED
- Certifications: BLS - Basic Life Support Required Within 3 months of hire or transfer, MA CERT - Medical Assistant Required
- Skills: Ability to be empathetic and resourceful to all patients regardless of their social and/or medical complexities, multi-tasking skills, ability to facilitate messages, complaints, and urgent calls as appropriate, ability to work collaboratively with internal/external healthcare team members and navigate through the healthcare system.
Exposure to stressful situations, including those involving public contact, as well as, trauma, grief, and death. Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.
