About the Role:

You will work as part of an integrated treatment team, providing comprehensive case management and care coordination services to adults with mental illness. Your primary responsibilities will include:

  1. Assessing client needs and developing individualized plans to support their recovery goals.
  2. Coordinating services and referrals to ensure seamless care delivery.
  3. Facilitating Recovery Team Meetings to promote collaboration and communication among team members.

About the Position:

  • Full-time position with flexible scheduling options available.
  • Location: Minneapolis, MN.
  • Estimated Salary: $45,760-$47,840 per year.

About You:

  • Bachelor's degree in one of the behavioral sciences or related fields.
  • At least 2,000 hours of supervised experience in delivering services to adults or children with mental illness, substance use disorder, or emotional disturbance.
  • Training on mental health recovery, de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effects.
