About the Role

The Disability Care Coordinator will provide individualized home support with training to assist individuals in achieving their personal community goals. This includes but is not limited to, personal hygiene, meal prepping, community engagement and integration, and medical appointment management.

You must be at least 18 years old, have a high school diploma/GED, a valid driver's license with a reliable vehicle, and proof of up-to-date auto insurance. Prior experience working with individuals who have developmental disabilities in a social services field is preferred.
