About the Role:
- Manage and mentor a team of Nurse Navigators working in Adult Primary Care
- Coordinate care for patient populations and facilitate one call resolution of patient needs
- Partner with Primary Care providers to deliver optimal non-patient facing care via telephonic and/or electronic means
- Develop and implement effective care coordination plans

Requirements:
- 3-5 years of experience in a similar role
- Master's Degree in Nursing or related field
- Active RN license
