Medical Care Manager

We are seeking a highly skilled Medical Care Manager to join our team at STARS Adult Medical Day Care Center. In this role, you will be responsible for creating and managing personalized care plans for individual patients.
About the RoleThis is an exciting opportunity for a dedicated Registered Nurse (RN) who is passionate about delivering exceptional patient care. You will work closely with our interdisciplinary team to assess, plan, implement, and evaluate patient care plans.
Responsibilities- Care Planning: Coordinate with colleagues to assess, plan, implement, or evaluate patient care plans
- Quarterly Conferences: Conduct quarterly care planning conferences for each participant enrolled
- Nursing Care: Provide nursing care based upon physician's orders and daily assessment of each participant's health status
- Staff Education: Participate in staff education activities and provide consultation facility personnel
- Record-Keeping: Maintain accurate records of patient care and discuss observations with the supervisor
- Medication Administration: Administer medications and monitor patients and manage coordination of care
- Patient Communication: Document any changes in patient conditions and report on changes
- Licensure: Currently licensed Registered Nurse (RN) in state of employment
- Experience: Previous experience as a Registered Nurse is preferred
- Skills: Excellent interpersonal and client care skills
- Certification: Valid driver's license and First aid/CPR certification required
