Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Wellstar Health System. As a Nurse Care Coordinator, you will play a vital role in delivering high-quality patient care and promoting a positive patient experience.

Key Responsibilities
  • Provide Patient-Centered Care: Deliver individualized care to patients, coordinating with the healthcare team to ensure seamless care transitions and optimal patient outcomes.
  • Collaborate with Interdisciplinary Teams: Work closely with physicians, nurses, and other healthcare professionals to develop and implement effective care plans.
  • Communicate Effectively: Maintain open and transparent communication with patients, families, and healthcare teams to ensure informed decision-making and timely care.
  • Participate in Performance Improvement: Contribute to quality improvement initiatives, identifying areas for improvement and implementing evidence-based practices to enhance patient care.
  • Develop and Maintain Clinical Expertise: Stay current with best practices and industry developments, applying knowledge to improve patient outcomes and advance nursing practice.
Requirements
  • Education: Graduate of an accredited/approved school of nursing.
  • Licensure: Current RN licensure in the state of Georgia or eligible for multi-state compact.
  • Certifications: Basic Life Support (BLS) certification required; BLS Instructor certification preferred.
  • Skills: Strong communication and interpersonal skills; ability to work effectively in a fast-paced environment; proficiency in electronic medical records (EMRs) and other healthcare technologies.
