- Position Overview: As a Registered Nurse specializing in Care Coordination, you will evaluate the needs of patients and their families, collaborate with the Interdisciplinary Group (IDG) to develop comprehensive Care Plans, and deliver compassionate palliative and supportive care.
- Required Qualifications:
- Active RN license in the state of employment.
- At least one (1) year of nursing experience in an inpatient or hospice setting for external candidates; internal applicants must have a minimum of 6 months of LPN experience in hospice or home health.
- Familiarity with EMR/EHR systems.
- Possession of a valid driver's license and automobile insurance as per company policy.
- Reliable transportation to fulfill visit schedules.
- Proficient communication skills in English, both verbal and written.
- Willingness to work a flexible schedule to accommodate the needs of hospice patients.
- Ability to manage the emotional challenges associated with caring for terminally ill patients and their families.
- Capability to access various patient environments, including navigating stairs and confined spaces, while meeting the physical demands of the role.
- Current BLS certification for healthcare professionals.
Core Competencies: Successfully complete all competency requirements for this role.
General Responsibilities:
- Represent Chapters Health System with professionalism at all times.
- Adhere to all regulations and company policies.
- Follow health, safety, and security protocols.
- Maintain confidentiality regarding patients and colleagues.
- Utilize resources responsibly and promote the organization.
Key Responsibilities:
- Deliver direct care to patients and families as part of the Interdisciplinary Group.
- Develop personalized Care Plans and provide education to patients and families.
- Monitor changes in patient conditions and engage in hospice referral and admission evaluations.
- Facilitate bereavement support and maintain effective communication with the team.
- Oversee care provided by team members and perform additional assigned tasks.
Physical Requirements: Ability to perform essential job functions with or without reasonable accommodation, including visual, auditory, and physical tasks such as walking, standing, lifting, and bending.
Drug and Alcohol Policy: Consent to drug and alcohol testing following a conditional offer of employment and comply with the Drug-Free Workplace Policy.
