Position Overview: The Senior Care Coordinator RN is tasked with evaluating transitional care requirements, orchestrating care throughout the healthcare continuum, and collaborating with patients and their families to ensure that the most intricate needs are addressed during the patient's acute care phase. This role necessitates advanced clinical knowledge to work alongside physicians and care teams to develop optimal and efficient treatment strategies that facilitate the patient's care journey while hospitalized, as well as preparing for post-discharge arrangements.

The Senior Care Coordinator RN will act as a key resource and advisor to team members regarding care progression and will assist in planning to effectively meet patient needs, manage length of stay, and promote the efficient use of resources. The role encompasses the integration and coordination of care facilitation, care progression, and transitional care planning functions.

Key Responsibilities:

  • Transitional Care Planning: Conduct assessments based on initial screenings to identify psychosocial risk factors and resource availability for discharge.
  • Collaboration: Work closely with financial counselors and utilization management nurses to evaluate insurance and coverage requirements for all payers, ensuring compliance.
  • Care Progression: Engage with physicians and the care team to facilitate communication regarding patient care progression, ensuring timely and effective care delivery.
  • Discharge Planning: Oversee all aspects of discharge planning for assigned patients, developing individualized plans in collaboration with the care team.
  • Documentation: Maintain accurate and timely documentation of clinical assessments and interactions with patients, families, and care teams.
  • Mentorship: Assist in the orientation and training of new staff, providing guidance in clinical progression and case reviews.

Qualifications:

  • Education: Bachelor’s degree in Nursing.
  • Licensure: Registered Nurse (Single State) or RN - Multi-state Compact, Basic Life Support (BLS) certification, and Certified Case Manager (CCM) or Accredited Case Manager (ACM) preferred.
  • Experience: Minimum of 3 years as a staff nurse in an acute care setting and 2 years as a case manager in a hospital or payer-based model.
  • Skills: Exceptional written and verbal communication skills, strong assessment and organizational abilities, and knowledge of local, state, and federal regulations.
