Job Summary:

We are seeking a Medical Social Worker to join our team in San Diego, CA. The successful candidate will be responsible for implementing standards of care for medical social work services in a home health setting.

Key Responsibilities:

  • Develop and implement individualized care plans to meet the unique needs of patients and their families.
  • Conduct thorough assessments of patients' social and emotional needs, and provide supportive counseling and guidance as needed.
  • Collaborate with healthcare professionals to ensure comprehensive care and coordination of services.
  • Provide education and support to patients and their families on health-related issues and resources.
  • Document all interactions and interventions in a timely and accurate manner.

Requirements:

  • Possesses a degree from a baccalaureate or master's program in social work accredited by the Council on Social Work education.
  • One (1) year experience in healthcare, preferably in a home health setting.
  • Basic Life Support (BLS) through the American Heart Association (AHA).

Preferred Qualifications:

  • Home healthcare experience.

Compensation:

$40 - $45 per hour.
