- Coordinate Patient Care: Collaborate with interdisciplinary teams to ensure seamless patient care and effective discharge planning.
- Case Management: Provide ongoing support and guidance to patients and families, connecting them with community resources and services as needed.
- Leadership and Education: Serve as a role model and consultant to new staff and students, providing education and training on clinical best practices.
- Master's Degree in Social Work: From an accredited graduate program.
- 1 Year Experience in Behavioral Health: In a clinical setting.
- California BBS Licensed Clinical Social Worker (LCSW): Required.
- AHA Basic Life Support for Healthcare Professionals: Required.
- 3 Years Experience in Behavioral Health: In a clinical setting.
- Acute/Psychiatric Hospital Experience: Discharge planning and case management experience.
