We are seeking a highly skilled Licensed Clinical Social Worker to join our team at BrightSpring Health Services. As a Licensed Clinical Social Worker, you will play a vital role in providing counseling services to patients in their natural home and community environment.
Responsibilities- Conduct comprehensive evaluations to assess the social, emotional, and support needs of patients and their families.
- Develop measurable objectives in conjunction with the rehab team to support patient recovery.
- Collaborate with the physician and other health team members to understand the significant social and emotional factors related to patient health problems.
- Provide instructive and counseling services to patients and families to treat and cope with social and emotional responses connected with illnesses.
- Participate in care coordination activities and act as a resource to other health team members in identifying and resolving patient needs.
- Identify and assist patients and families in utilizing appropriate community resources to achieve measurable objectives.
- Master's Degree in Clinical Social Work (LCSW)
- Current Licensed Clinical Social Worker licensure in the state of practice
- Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury, or acute diagnosis
- CPR certification required
- Ability to lift 50 pounds and perform physical demands of the job
